Geriatric Care Managers

Geriatric care managers, also known as aging life care professionals, are specialists who act as guides and advocate for families who are caring for older relatives or disabled adults by assessing needs and creating a care plan to assist in arranging housing, medical, social, financial and other services.

Among the tasks the Aging Care Life Association, formerly known as the National Association of Professional Geriatric Care Managers, notes that aging life care professionals or GCMs can perform on a fee-for-service basis are assessments to identify problems, eligibility for assistance and need for services; screening, arranging and monitoring in-home help or other services; assisting with moving a senior person into or from a retirement complex, care home or nursing home; acting as a liaison to families at a distance; reviewing financial, legal or medical issues and offering referrals to appropriate specialists; and providing counsel and support.
