JFS Ally Navigator – Helping LGBT+ Older Adults Access Medical Care

JFS Patient Navigator Program

If a caregiver cannot accompany their loved one appointments, there are some transportation resources that do provide someone to escort the older adult throughout the trip. One such resource is the Patient Navigator program offered by Jewish Family Service of Metrowest (JFS) Elder Care Solutions. 

JFS Patient Navigator is a nationally recognized program that uses trained and supervised volunteers to improve health care access and equity for low-income, frail and isolated older adults.   

JFS Patient Navigators help local older adults get to and manage their medical appointments, reduce social isolation, and assist with prescriptions. Additionally, the program provides assessment, referral and on-going case management when necessary and appropriate. No other program in MetroWest provides this combination of services.

Are you a member of the LGBT+ community and are worried about seeking health care for age-related issues? Do you have new or worsening symptoms but are nervous about going to the doctor alone? Join the JFS Ally Navigator program and bring an Ally with you!

LGBT+ Ally Patient Navigator Expansion

With support from the Tufts Health Plan Foundation Momentum Fund and the Cummings Foundations Sustaining Grant, the JFS Patient Navigator program has expanded its reach into the local older adult LGBT+ community to combat the health disparities that effect LGBT+ older adults and to break down the barriers that these individuals face in health care settings.

The program will partner with Fenway Health to design and to provide culture-competence training to Navigators and local medical providers, so that LGBT+ older adults, as all MetroWest older adults, can age in good health and with pride.

JFS Ally Navigators are now provided with additional training specific to the health needs of LGBT+ older adults. Ally Navigators will:

  • Ask about and respect patients’ varied wishes regarding their identity and their medical care
  • Help patients prepare for logistic concerns related to check- in or intake procedures
  • Soothe patient worries about seeing a new or less familiar provider
  • Serve as a buffer for the patient during conversations with staff
  • Seek to empower LGBT+ older adults to make informed decisions about their health care.
